Current guidelines for infant cardiopulmonary resuscitation (CPR) recommend two chest compression techniques: the two-finger (TF) for a single rescuer and the two-thumb encircling (TT) chest compression for two or more rescuers [4,5].

Encircle the infant’s chest so that the fingers of both hands support the infant’s back. Use your thumbs to deliver compressions at the appropriate depth and rate (100-120 compressions/minute; 1/3 of the depth of the chest or approximately 1 ½ inches).

If the second rescuer can fit their hands around the infant’s chest, perform CPR using the two thumb-encircling hands method (Figure 11g). Do not press on the bottom end of the sternum as this can cause injury to the infant.

Several recent studies have reported that the two-thumb-encircling hands technique is more effective for high-quality chest compression than the two-finger technique for a single rescuer performing infant cardiopulmonary resuscitation.

For a smaller child, you may use one hand, instead of two hands, to give compressions. For an infant (under 1 year of age), compress the chest about 1 ½ inches using the encircling thumbs technique (also known as the two-thumb/encircling hands technique).

The 2-thumb-encircling hands technique generates a significant higher ratio of correct compression position than the TF technique. Furthermore, the 2-thumb-encircling hands technique is less fatiguing than the TF technique.
