Accessory ossicles are supernumerary and inconstant structures that are not caused by fractures. Derived from unfused ossification centers, accessory ossicles were first described by Vesalius in 1543. For centuries, they were believed to be asymptomatic.

This accessory bone is found within one of the peroneal tendons off of the cuboid bone on the lateral (outside) portion of the foot. Os peroneum are fairly common and are found in about 20% of the population.

An os peroneum is a small bone that is embedded in the peroneus longus tendon as it wraps around the cuboid bone on the outside of the foot. Pain in this bone is known as os peroneum syndrome. A painful os peroenum is uncommon cause of chronic lateral foot discomfort.

This case highlights an exceptionally rare accessory ossicle called the os cuboideum secundarium, located adjacent to the cuboid and calcaneus. Interestingly, this case demonstrates the potential for this rare ossicle to mimic a mass on magnetic resonance imaging (MRI).
